This is a 28 nodes cluster running 11.2.0.2 on linux 5.5 64 bit
which has being happening as early as April 2011.
on April 18 customer applied patch 10121589 (in merge Patch 10387939) to fix
the error but still the customer has been hitting this. Recent occurance is
on
April 17th.
This to confirm patch 10121589 is installed.
